Game Story:

The player takes on the role of investigative journalist Miles Upshur as he navigates a run-down mental hospital in Leadville, Colorado, that is overflowing with murder patients. The player can walk, run, crouch, jump, climb stairs and jump over objects. However, unlike most games, the player has no visible health bar on the screen and cannot attack enemies.

The game often uses traditional fear jumps and audio cues to alert players when an enemy detects them. When the player records certain events with their camcorder, Miles writes a note about them that includes more information and thoughts. Documents be collected that provide background and other information about the facility, including pages from patient diaries and reports from hospital staff.

Developer Red Barrels has identified survival gameplay in Amnesia: The Dark Descent significantly influencing Outlast’s non-combat narrative style. Found footage horror films like Quarantine and REC also served as influences.
